About Form 12mo
When is it due?
Shortly after your 12-month mark — schools generally expect it within about ten days. Your DSO sets the exact expectation, so confirm with them.
Who signs it?
You write and sign it, and your employer official signs it too. You can send them a link for their part.
What if I already used OPTPrep for my I-983?
Then your details are already there. The evaluation is attached to the plan it belongs to, so you are not filling the same answers in twice.
